Day Habilitation & Vocational Direct Support Professional

 Essential Requirements:

  1. High school diploma or GED preferred.
  2. Valid driver’s license preferred.
  3. Computer experience preferred.
  4. Certification in developmental disabilities preferred.  Must be willing to meet certification requirements for the position.
  5. Must be able to perform CPR, AED, First Aid upon certification.
  6. Must be able to implement Therapeutic Response Techniques.
  7. Use proper body mechanics to reduce physical stress and fatigue in the work environment and prevent musculoskeletal disorders.
  8. Close vision, distance vision, peripheral vision, and depth perception required.

Major Responsibilities:

  1. Assist and support people to realize their goals and attain personal outcomes.
  2. Assist and support people to develop and/or maintain a natural support network.
  3. Assist and support people to develop and maintain social roles.
  4. Assist and support people to maintain their health and well-being.
  5. Assist and support people to utilize their community and its resources.
  6. Assist and support people to make informed choices in all aspects of their life.
  7. Assist and support people to live and work in a safe environment.
  8. Advocate for people with disabilities and teach members of the community to treat people with dignity and respect by modeling positive attitudes and respectful communication and interactions.
  9. Work as a member of a team to complete tasks as instructed, be receptive to new ideas, and be creative in solving problems.  Work cooperatively with respect to all team members and their contributions.
  10. Support people to achieve outcomes by providing opportunities to learn and grow regardless of their disability.  Provide training and support to help people recognize their rights and actively self-advocate.
  11. Maintain confidentiality of information about people served, records, personnel practices, internal practices, and the Agency’s business affairs.
  12. Promote teamwork within and between settings and maintain communication channels.  Consistently use people first language in all communication.
  13. Provide accurate, unbiased, and consistent documentation relating to training goals and medical/behavioral events on a daily basis.
  14. Display a positive and professional attitude in your interactions with people served, other employees, and other persons with whom your work for that Open Door Center brings you into contact.
  15. Consistently incorporate the Agency’s philosophy in daily services that meet state licensure and regulation requirements as well as Agency determined expectations.
  16. Abide by all Open Door Center policies and procedures.

Specific Duties and Responsibilities:

  1. Work hours assigned. If an emergency (accident, illness, hospitalization, death in family) occurs, notify the Supervisor with as much notice as possible prior to the start of the shift.
  2. Attend in-services and workshops as appropriate and approved. Attend meetings as requested.  Serve on Agency committees as assigned.
  3. Provide assistance to complete hygiene, grooming, and clothing care. Assist people to maintain their area and assure people have a safe environment.
  4. Assist in planning activities/tasks for people served on a daily basis based on preferences of the people supported. Assist people to access their community and use its resources.
  5. Encourage people to participate in opportunities that facilitate assertiveness and self-esteem. Assist people to fully exercise their rights and make choices.
  6. Assist people to celebrate the achievement of personal successes/milestones.
  7. Participate in the personal planning process by providing information to the team about what you know to be the dreams and goals, strengths, needs and preferences of people you support.
  8. Ensure all necessary agency forms and assessments are completed accurately. Follow all emergency procedures and drills. Assure proper documentation in Therap is completed for the people supported.
  9. Provide people supported with training and support to maintain their health. Assist with medication administration and follow medical provider’s recommendations.  Prepare well-balanced meals and snacks following each person’s diet orders.
  10. Assist people to learn and perform their job duties. Make accommodations when necessary or required.  Maintain any adaptive equipment utilized by the person supported to continue employment.
  11. Assist people supported with interpersonal skills necessary in the workplace. Observe the abilities of the person supported and provide feedback and assistance in any areas in need of improvement.
  12. Maintain positive working relationships between the agency and area businesses.
